The Well app is a health network connecting patients with providers, and providers with each other, built around patients' own portable, shareable medical records. Patients capture their symptoms, history, and lifestyle through an intelligent virtual companion, then share a complete record with any provider. Doctors get an organized, AI-generated history in hand—plus AI-assisted summaries, diagnoses, and documentation. The result: less paperwork, more continuity, and coordinated care.

Your data the app uses

Patient health records retrieved via Fasten Health over the TEFCA network (from CMS-Aligned Network QHINs), returned as FHIR R4 / US Core resources; identity verification via CLEAR or ID.me (through Fasten) at NIST IAL2/AAL2; and patient-provided data (self-entered symptoms and notes, uploaded documents, and connected device/wearable data).

  • Manage your health records

    Well brings your health records together from the different providers, hospitals, and networks that care for you, so your information lives in one application on your phone. You can view your conditions, medications, allergies, lab results, immunizations, and visit summaries without logging into separate patient portals. Your records are gathered securely after you verify your identity once, and you stay in control of what is collected. It's an easy way to see your full health picture instead of pieces scattered across many systems.

  • Use AI to get insights on your health

    Well's opt-in AI assistant uses your own health information to give you personalized, easy-to-understand guidance. It can help you make sense of your records, understand symptoms, and prepare questions before a visit. Every AI response is clearly labeled as AI-generated and reminds you that it is educational and not a substitute for advice from your doctor. You turn the assistant on yourself, and it only uses the information you choose to share with it.

  • Track medication

    Well keeps your medications in one application, drawn from your health records and any you add yourself. You can see what you take, review it before appointments, and share an up-to-date list with anew provider. An accurate medication list in one spot helps you and your care team avoid gaps and duplication. Well helps you keep track of your medications; it does not prescribe or change them.

  • Get health coaching

    Well offers personalized, education-based guidance to help you stay on top of your health between visits. Using your own information, it can help you understand your care plan, prepare for appointments, and know what to ask your provider. This guidance is educational and clearly labeled — it supports, but does not replace, the advice of your doctor or care team. You choose to use it, and you control the information it draws on.

  • Share information with caregivers or providers

    When you visit a provider, Well lets you share a summary of your health information securely — such as with a simple QR code — so you don't have to fill out clipboard forms or repeat your history. You decide exactly what to share, with whom, and when, and you can also share with a family member or caregiver who helps manage your care. Sharing happens only with your proactive consent, and links are protected and time-limited. This keeps each visit smoother while you stay in control of your information.
