Triangle Health is a free personal health app for Medicare enrollees. Connect your Medicare account to bring your claims — hospital, doctor visits, and prescriptions — together with records you upload, all in one secure application. A built-in AI assistant helps you understand your health history in plain language, and you can share it with family and your care team. Optional clinical services, such as deeper record retrieval, telehealth visits, and lab testing, are available if you want them.

Your data the app uses

With the beneficiary's authorization, Triangle accesses three kinds of health information. First, Medicare claims—records of hospital care (Part A), doctor and outpatient visits (Part B), and prescriptions (Part D)—connected through the CMS Blue Button API after the beneficiary signs in and consents on Medicare.gov. Second, clinical records from the beneficiary's hospitals, clinics, and labs, retrieved through our connectivity partner, Particle Health. Third, documents and information the beneficiary uploads or enters directly. Within the app, this information is organized into one clear, searchable health record. Triangle's AI assistant uses it to generate plain-language summaries, explain medical terms, and answer questions about the beneficiary's history. This content is always labeled as AI-generated and educational, not medical advice. Beneficiaries can maintain a medication list, prepare for appointments, and share their records with chosen family or care-team members. Health information is stored in secure, U.S.-based cloud infrastructure and encrypted both in transit and at rest. Access is limited by role, ensuring staff only see the data needed to operate the service. Triangle never sells beneficiaries' health information and does not share it with third parties for advertising or marketing. AI processing is performed by vendors under zerodata-retention agreements, meaning they do not keep the data or use it to train their models. Beneficiaries remain in control: they can disconnect any data source at any time to stop new collection, and they can delete their records or their entire account.   • Manage your health records

    Your Medicare claims and medical records in one secure place. Connect your Medicare account to pull in hospital visits, doctor visits, and prescriptions, and add your own documents alongside them. Everything is encrypted and stored in the U.S., and you can disconnect a source or delete your records at any time. 

  • Use AI to get insights on your health

    A conversational assistant answers questions about your records and medications in plain language, and summarizes documents or your history as a whole. It works only over your own records. Every response is labeled as AI-generated and educational, not medical advice, and prompts you to confirm anything important with a clinician. It does not diagnose or prescribe. 

  • Track medication

    Triangle builds your medication list from the prescription records connected to your account, so you can see what you have been prescribed without typing it in. Keep it current as things change, and have it ready for appointments or a new clinician. Share it with family or your care team along with the rest of your records. 

  • Get health coaching

    Triangle helps you prepare for appointments and understand what your records mean, so you arrive with better questions. The assistant walks you through documents in plain language and helps you organize what to raise with your clinician. This is educational support, not a substitute for professional care. Optional telehealth visits and lab testing are available if you want more. 

  • Share information with caregivers or providers

    Share your records with family, caregivers, or care team members you choose, and revoke access at any time. A family member can help manage your care, or a new clinician can see your history without starting over. Triangle never sells your health information or shares it for advertising.
