Your data the app uses

HealthScores.ai accesses a beneficiary’s medical information only with the beneficiary’s authorization. Depending on the healthcare connections and records the beneficiary chooses to add, this information may include medical conditions, diagnoses, medications, allergies, procedures, encounters, laboratory results, vital signs, immunizations, clinical notes, care plans, advance directives, and medical documents obtained from connected patient portals or uploaded by the beneficiary. HealthScores.ai organizes this medical information into a patient-controlled health record that helps the beneficiary understand and manage their health, prepare for appointments, complete medical forms, coordinate care, and make essential information available during an emergency. The app may create summaries and allow the beneficiary to share selected medical information with authorized caregivers, healthcare practitioners, pharmacists, or emergency personnel through approved methods such as QR-based access, SMART Health Links, FHIRrecords, or PDF documents. A central component of HealthScores.ai is its use of the beneficiary’s Google drive account. With the beneficiary’s permission, medical records and documents are securely written to and accessed from a designated HealthScores.ai folder within the beneficiary’s Google drive account. This allows the beneficiary’s Google drive account to serve as the primary repository for their medical records rather than requiring HealthScores.ai to maintain a separate long-term copy of the complete record. The beneficiary retains control over Google drive account access, file permissions, sharing, and retention. HealthScores.ai uses encrypted transmission, authorized Google API access, least-privilege permissions, role-based access controls, audit logging, data minimization, and incident-response procedures to protect medical information processed through the application.

This allows the member to manage their health information as a reusable profile instead of leaving it scattered across portals, paper forms, and disconnected systems.

    HealthScores.ai uses AI to generate a plain-language summary of the member's most important health information. The summary can highlight active conditions, current medications, allergies, recent encounters, care instructions, insurance details, and emergency-relevant information. This helps the member, caregiver, emergency responder, or provider quickly understand the member's health context without having to navigate multiple portals or documents. The AI summary is designed to support communication and preparedness, not replace medical judgment.

  • Connect to wearable devices

    HealthScores.ai connects a member's wearable device data to their patient-controlled health profile so daily health activity is no longer separated from the rest of their medical information. With permission-based connections, the member can authorize HealthScores.ai to receive wearable data, including activity, steps, sleep, heart rate, oxygen saturation, and other supported device signals. This allows HealthScores.ai to combine lifestyle and biometric trends with the member's medications, conditions, allergies, care instructions, emergency contacts, and other health records. HealthScores.ai's security documentation supports this type of workflow by allowing data from approved third-party integrations to be processed through authorized application services with access controls, validation rules, audit logging, and transmission controls.

    HealthScores.ai allows a member to invite trusted caregivers, family members, or support contacts into their Care Circle. This gives caregivers access to the health information the member chooses to share, such as medications, allergies, conditions, emergency contacts, advance directives, provider information, or care instructions. The goal is to help caregivers stay informed, coordinate support, and respond more quickly when the member needs help. Sharing is designed to follow approved consent, authorization, and access-control workflows.
