Description

Principal care management are disease-specific services to help manage your care for a single, complex chronic condition that puts you at risk of hospitalization, physical or cognitive decline, or death. Your provider will create a disease-specific care plan and continuously monitor and update it, including any changes to the medicines you take.

Who's eligible

 If you have one chronic high-risk condition that you expect to last at least 3 months (like cancer) and you aren't being treated for any other complex conditions, Part B may pay for a health care provider to help manage it.

Costs

After the Part B deductible , you pay 20% of the Medicare-approved amount .

Ask your doctor or healthcare provider how much your test, item, or service will cost.

Your doctor may recommend services that Medicare does not cover or offers too frequently. This could end up in additional costs for you. Make sure to ask your doctor about the reasons for these recommendations and what Medicare will actually cover.

  • How much your doctor charges

  • If your doctor accepts assignment

  • The type of facility

  • Where you get your test, item, or service
