If your drug is on the list, click on the link labeled "details" for detailed information about the program. Please note that Pharmaceutical Assistance Programs offered by Pharmaceutical Companies generally cannot coordinate the assistance they provide with your Medicare benefits. The value of any assistance you receive from a Pharmaceutical Assistance Program for your drug(s) will not count toward your out-of-pocket costs or total spending under your Part D benefit.

| Eligibility Criteria | - Must be a citizen of the United States or its Territories.
- While Part D enrollees are generally not eligible, exceptions are possible with a doctor’s letter of necessity and eligibility is determined based on insurance and income eligibility criteria.
- Part D enrollees who pay up to 3% in Out-of-pocket fees for medication may be eligible.
- Cannot have coverage for the covered product.
- The patient must have an annual income less than or equal to 200% of the Federal Poverty Guidelines, $21,660 for individuals and $29,140 for couples.
