Phone Numbers

Issues:

  • Missing Phone Number
  • Incorrect Phone Numbers that:
    • Include Extensions
    • Are Out of Date
    • Are Completely Incorrect
  • Duplicate Phone Numbers

Recommended Action:

Updates can be made by using Internet-based PECOS or the paper-based enrollment process.


  1. Internet-based PECOS:
    • You will be able to add, change or delete phone numbers via Internet-based PECOS
  2. Paper Process:
    • Complete, sign, and submit the CMS-855I and supporting documentation to your Medicare contractor.
    • Fill out sections 1A -changing information, 1B – practice location information, 2A, 3, 4 (complete only those sections changing),13 and 15 based on the phone number information you would like to update.
    • If you practice at an address in which they have reassigned your benefits, you must ensure that the CMS-855R and CMS-855B forms have been filled out and processed through the Medicare contractor in their area.


Issues:

  • Home Phone is Listed
  • Fax Number Listed instead of Phone Number

Recommended Action:

  1. Notify CMS by using our feedback tool. When sending updated information, please include name, specialty, practice location address(es), and any available contact information. CMS will automatically remove information from the Web site.
  2. You will also need to update your Medicare enrollment records. Updates can be made by paper process or by Internet-based PECOS
