What are the Physician Group Practice Demonstration and the Medicare Care Management Performance Demonstration?

Over 660 physician practices of all sizes have participated in the demonstrations that reward physician groups for improving and delivering high quality health care. Physicians participating in the demonstrations submit quality measures focusing on chronic conditions (such as diabetes, heart failure, and coronary artery disease) and preventive care (such as hypertension, immunizations for flu and pneumonia, and cancer screening). These targeted conditions are highly relevant to the Medicare population. Each physician group's annual results are compared to benchmarks, and groups that meet the benchmarks are eligible for performance-based incentive payments.
