Updates can be made by using Internet-based PECOS or the paper-based enrollment process.

1. Internet-based PECOS:
• Names can be changed via Internet-based PECOS, however, your updated name will have to match the legal name on file with Social Security Administration.
2. Paper Process:
   • Complete, sign, and submit the CMS-855I and supporting documentation to your Medicare contractor.
   • Complete Sections 1A -changing information, 1B – identifying information, 2A – personal information, 3, 13, and 15 in the form based on the information you would like to update.
