surcharge on your Part B premium based on the 2009 premium amounts. If
you are under age 65, disabled or if you had Part B before and dropped
it, please contact Social Security at 1-800-772-1213 for
specific information about your Part B premium amounts including any
surcharges. If you didn’t take Medicare Part B when you were first eligible
because you or your spouse were working and had group health plan coverage
through your or your spouse’s employer or union, please contact Social
Security at 1-800-772-1213 for specific information about your
Part B premium amounts including any surcharges.
